What I do

I take an AJA HD10A and send it the YPbPr signal from the HDR or HVR analog outputs which makes it HD-SDI. I send the analog audio around it to the 1200, 3700 or F500 and select "line level audio"...

The official and only model number (as is distributed by authorized dealers in the US) is JY-HD10U. There is a consumer version as well but I only carry and use the JY-HD10U. Other world markets...
